Here is Thomas R Mooney's obituary: Thomas R. Mooney, 67 of Chula Vista, CA passed away on June 11, 2012. He was born in Elizabeth, New Jersey on March 20, 1945. Tom graduated from Brick Township High School in Brick New Jersey. He graduated from Park College in Parkville, Missouri in 1967. After college Tom applied for Navy Officer Candidate School, in Newport RI in 1968. In 1970 Tom was promoted to Lieutenant Junior Grade. Here is the basic form letter for requesting a new Fletcher for the fleet.

Basically you can copy from this post, place text in a word document, make the changes that apply to your case and then mail it to the address provided.

Start the copying from the next line down.
